DESCRIPTION:

JAC Planning Corp. performed a expanded Environmental Assessment Form (EAF) to receive a determination of non-significance or “Negative Declaration” and thus achieved and abbreviated environmental process with the Village of Rockville Centre.  In addition to the EAF JAC Planning prepared the following:

	Description of Mercy Hospital and Land Use
	Description of surrounding zoning and land uses and the compatibility of the proposed project
	Detailed project description and narrative
	Justification of why the project is needed (all C.O.N. applications) and the community benefits
	Water consumption and sewage generation calculations
	Traffic analysis that was prepared by a traffic engineering consultant
